Has anyone created a form fillable PDF of the clan sheet for use with Heroquest 2 (e.g., the one from the Kingdom of Sartar book)?

There is a Sartar clan generator here (thanks to Charles Corrigan): http://www.glorantha.com/site-apps/clan/sartar.html
It allows you to walk through the questions online and generate the result.
